Riverside Community Diabetes Collaborative The Riverside Community Diabetes Collaborative is pleased to bring you this directory of diabetes care and prevention resources for the City of Riverside and surrounding communities. We hope that this directory will help you access the services and resources you need to prevent diabetes and to help those with diabetes to find care and support services to live healthy, active lives. About RCDC The Riverside Community Diabetes Collaborative was formed in February of 2007 in response to the increasing number of persons diagnosed and at risk for diabetes. We are a group of diverse organizations and individuals with a desire to make an impact in the fight against diabetes. Our mission is to strengthen community programs and services for the prevention and management of diabetes. Our goal is to identify community needs to effectively address the gaps in services in order to decrease the onset of new diabetes cases and the long-term complications of the disease.

Did You Know? • Nationwide o Nationwide, 20.8 million children and adults—7 % of the population—have diabetes. Of these, 14.6 million people are diagnosed, and 6.2 million people are undiagnosed. 54 million people are estimated to have pre-diabetes.1 o Diabetes costs Americans $174 billion annually, a figure that has increased by 32 percent since 2002.2 • Countywide o The prevalence of diabetes in the Riverside County adult population is estimated to be 6 %, or roughly 74,000 adults, with nearly half over the age of 65. 4 o 16% of the county’s children aged 12-17 years are overweight or obese.4 o Roughly $21 million was spent on treating uncontrolled diabetes at Riverside County hospitals in 2004, compared to $13 million in 2001.4 • Citywide o From 2003 to 2005, there were 768 hospitalizations for uncontrolled diabetes among residents of Riverside City, a rate of 192.5 hospitalizations per 100,000 population.5 o In 2005, there were 85 diabetes-related deaths in the City of Riverside, with a mortality rate of 21.6 per 100,000 population.6

Please read the descriptions for each program below: A)“Follow the Leader: Guiding Families to Healthy Living”. A six-week course, designed for parents with young children (0-5), covering the topics of obesity, the consequences of obesity, diabetes, nutrition, and physical activity. Sessions are conducted at area Head Start and pre-school program sites. Call for more information. B) “Por Tu Familia (For Your Family)”. A fun, activity-based, educational program designed specifically for the Hispanic/Latino community about the prevention and management of diabetes. This program is taught in Spanish and English by promotoras (community outreach workers) and utilizes curriculum from the American Diabetes Association. Glucometers will be provided based on insurance (provided that the patient attends both sessions). By appointment only. (2) Diabetes Self-Management . One-on-one diabetes teaching specifically for a patient’s need such as diet and meal planning, medication, insulin administration, and dose adjustment.

FOOD DIABETES When you live with diabetes, it's important to take extra care to make sure the foods you eat are kept in balance with physical activity, as well as any diabetes medications your doctor may have prescribed. But, that shouldn't stop you from enjoying the same delicious meals as people who don't have diabetes.

Here are some general nutrition tips for achieving good blood sugar control: o Eat a variety of foods from all of the food groups o Eat a meal or snack at least 3 times a day o Eat smaller portion sizes o Choose low fat meat and dairy products o Choose whole grains over white flour products and whole fruits/vegetables over juices o Choose water and sugar-free beverages more often

Carbohydrates, Protein, Fat and Calories

Carbohydrate is your body's main source of fuel and will have a direct effect on raising your blood sugar. Carbohydrates are found in fruits, starchy vegetables, grains, beans, dairy products like milk and yogurt, and of course, sweets and sugary beverages. You'll want to consume the right amount of carbohydrate at each meal to avoid blood sugar swings from high to low. A registered dietitian or member of your diabetes care team can determine how much carbohydrate you should include at meals and snacks and can teach you how to count carbohydrates in your foods. One carbohydrate (carb) choice is a food portion with 15 grams of carbohydrate. Most women need 3-5 carb choices per meal and 1-2 per snack. Most men require 4-5 carb choices per meal and 1-2 per snack. Carbohydrate content of foods can be found on food labels and on food lists. Protein and fat have less impact on blood sugar levels but it is important to eat small portions and choose low fat foods to limit excess fat and calories, which can cause weight gain and lead to diabetes complications. Some types of fat are better for you than others. For example, the fat in fish, nuts, avocado, canola and olive oil are less likely to cause your blood vessels to clog than the fat in red meat, butter or lard. In general, non starchy vegetables (all raw vegetables and all cooked vegetables except corn, peas, potatoes and winter squash) are very low in carbohydrate, fat and calories and can be eaten freely. BUILD YOUR PLATE:

To give you a visual of how all these suggestions fit together in the form of an actual meal, we have applied them to a plate. By following this plate method, you can plan a low fat, high fiber, low calorie meal and keep your carbohydrate intake somewhat consistent, without counting grams or calories.

Use a 9 inch plate (this in itself will help you control portions).

For Lunch and Dinner: o Fill ½ of your plate with non starchy vegetables such as carrots, broccoli and salad. o Fill ¼ of your plate with a starchy choice such as rice, pasta, corn tortilla, corn, peas or potato. o Fill ¼ of your plate with lean meat, chicken or fish; this would be about 3 ounces (or the size of a deck of cards). Plant proteins like beans, peanut butter and tofu can be substituted for animal proteins. o Add 1 small fruit or ½ cup of cut up fruit as a side dish or dessert o Choose 1 cup of 1% milk as a beverage

Physical Activity in general can lower blood glucose as much as 50 points in women and 70 points in men during the activity, even when the activity is only done now and then. Regular physical activity can result in further improvement of blood sugar all day long. It works much like insulin or other diabetes medicines. That's wonderful news because many people have learned to control their blood sugars without the use of medicine or with less medicine by exercising and healthy eating alone. Physical activity pulls sugar from the blood and helps the body cells use it as energy. This results in a lower blood sugar. The sugar, or energy, is burned during the physical activity you do. Even better news is that you don't have to be an athlete or run a marathon to get these results. Simple, everyday tasks like washing a car, gardening, cleaning house, dancing, or walking to the store can burn up extra sugar.

Other Great Reasons To Be Active Boost Your Energy: Although you may feel tired before your activity, you will feel more energized after the activity and all day long. Lift Your Mood: Reduce feelings of stress, loneliness, and anxiety. Sleep Soundly: Get the rest you need to feel good and enjoy each day. What Physical Activities Can You Do? There are lots of different types of activities you can do but basically each activity will fall into one of these categories: Stretching/Flexibility Aerobic

Balance Strength Training

Because aerobic exercise is simple, safe and very beneficial to all aspects of diabetes and health, you may want to start with that. If you are not active now, start slowly. Examples of Aerobic Exercises are walking, swimming, dancing and bicycling. How Do I Get Started? Pick an activity that you enjoy. Begin with 5-10 minutes a day for the first week.Increase by 3 minutes a day the next week. Stay at this level until you feel comfortable. Continue to slowly increase the time or the intensity of the activity with the goal of completing 30 minutes most days of the week. Check with your diabetes health care team as to specific instruction.

Exercise Safety Tips For People With Diabetes 1) Check with your doctor first. About any exercise limitations he/she has for you before you start. 2) Check your blood sugar before and after exercise. If your blood sugar is less than 100, eat a small snack before exercising. If your blood sugar is 300 or more, delay exercise until it comes down to a more normal level. 3) Be Prepared. Carry glucose tablets or another form of simple carbohydrate with you in case your blood sugar becomes low. 4) Carry ID. Wear identification that says you have diabetes such as an ID bracelet or shoe tag. 5) Stay hydrated. Drink Plenty of Fluids before, during and after exercise. This will help keep your body cool and prevent dehydration. 6) Pay attention to your feet. Check your feet before and after activities for any red spots, cuts or blisters. Wear comfortable and well-fitting shoes and socks.

What Can I Do To Prepare For Visits With My Health Care Team? First of all, be sure to schedule and keep regular appointments with your diabetes care team. Secondly, come prepared. The more you can tell your health care team about your health, the better they can respond and help you. 1. Make a list of your questions and concerns: As they come to mind so you won’t forget to ask. Be sure to leave enough room for answers so you don’t forget what was said. 2. Keep a blood sugar log: Writing it down makes it easier to identify patterns. Test blood sugar more often the week before your visit, record the blood sugar level and time checked. Record the time, quantity and type of food you ate, along with exercise type, start/end time. 4. Bring your medications with you: Be sure to bring any medications you take with you, including those prescribed by other health care providers. This way your doctor can review them all to be sure they are all compatible. 5. Keep a record of your exams: Be sure to mention if you have had exams or tests by specialists other than your diabetes care team. If possible, bring test results with you. 6. Schedule tests in advance of your visit: Ask your diabetes care team if there are any lab tests you need to have done before your visit. This will save time and allow you and your doctor to discuss the results in person when you meet. 7. Ask a family member or close friend to join you: Bringing someone you trust along will help you remember and record everything that is discussed.
