    To Tammert Aero AB., Kiruna, Sweden with new c/r SE-KPB.

    To Loon Air with new c/r C-GKPB.

    22 May 1993

    Crashed.
    Summary: The damage to the airframe was substantial. There were two fatalities. It was a local testing flight from Edmonton/Villeneuve Airport, AB (CZVL), Canada. The accident occurred at Edmonton/Villeneuve Airport, AB (Canada) while taking off. The airframe was repaired. Narrative: The DHC-3 Otter was being test-flown after re-assembly. After lifting off the runway it veered left, reaching a height of 75 feet. The left wing contacted the ground and the airplane crashed.

    Repair completed.
    Rebuilt at Courtenay, BC., by International Aero Products Inc using parts from c/n 350 -Converted to Pezetel power ASZ-621R-M18 -Baron STOL wing modifications and put on Wipline 8100 floats.

    Certificate of airworthiness for N6868B (DHC-3, 274) issued.

    28 July 2005

    Crashed.
    There was one serious injury. The accident occurred at Ketchikan, AK, United States while cruising. The damage was minor. The airline transport certificated pilot was conducting a visual flight rules (VFR) local area sightseeing flight. During cruise flight, a flash fire erupted near the windshield/glare shield area of the cockpit, in front of the pilot. The pilot made an emergency landing and a passenger assisted by utilizing a fire extinguisher to put out the fire...

    Restored.
    Rebuilt. The fuselage and wings are from 59-2212. The tail came from c/n 274, ex 4642 Myamar AF.
