AW139 Helicopter Crash on Monte Rosa

An AgustaWestland AW139 helicopter, operated by Avincis Aviation Italia, crashed during a rescue mission near the Capanna Margherita Refuge on the Monte Rosa summit, Italy, at an altitude above 4500 meters. The incident, occurring on 16 March 2024, involved four occupants (a pilot, an alpine rescue technician, a flight technician, and a dog handler), all of whom survived without severe injuries. Preliminary reports suggest strong wind gusts might have been a contributing factor to the crash; rescue operations were complicated by the challenging weather conditions, but all individuals were safely recovered by Air Zermatt helicopters. Boeing 787 Safety Advisory Issued

Following the March 11, 2024, upset of a LATAM Chile Boeing 787-9 over the Tasman Sea, resulting in 12 injuries, Boeing has released an advisory to all 787 operators. This notice, building on a 2017 advisory, addresses a potential issue with the pilot seat's rocker switch cap becoming loose or detached, which could lead to unintended seat movement. Boeing recommends applying adhesives to the switch caps and is evaluating updates to the Flight Crew Manual to mitigate this risk, part of ongoing safety enhancements following the incident investigated by Chile's aviation authority with input from New Zealand's TAIC and Australia's ATSB.
